Alternate Reality: The Dungeon directly follows its predecessor: The player is still captured by aliens and imprisoned in an alternate reality. After visiting the city, he now heads to the dungeon with only one goal: to survive.

While the predecessor focused more on exploring the city, this installment has a mostly linear path: the dungeon consists of four levels that must be visited in order. But the environment does not mean that there are only monsters to fight in the turn-based combat system or treasures to loot: the player still has the stores at his disposal, e. g. an inn or a blacksmith's shop. Even the six guilds (plus two additional ones) have found their place in the dungeon; this time the player can even become a member. A major change in gameplay is that all items have a weight associated with them: carrying too much is bad for speed and stamina.

The dungeon is separate from the city and after entering there is no turning back. Saved games can be imported, but the player loses all but sixteen potions.
