Fantastic Voyage is a side-scrolling shooter based on the 1966 movie of the same name. Scientist Jan Benes is about to die due to a blood clot in his brain. A submarine, controlled by the player, shrinks and has to reach the brain through three levels. The enemies, which lower the energy bar when touched, are human defense systems, such as antibody cells.
In addition to shooting enemies, there are also several passages where the player needs to guide the submarine through narrow corridors without touching the environment. Along the way, the player needs to collect nine circuit boards needed to destroy the blood cot; these may be hidden in dead ends or alternate paths. This means that the player can move forward and backward to try other routes at forks. It is important to collect oxygen and fuel canisters to keep the submarine running. Shield energy can be refilled by killing three enemies without missing a shot.
