The fourth installment in Legacy's Emergency Room series retains the same mechanics and premise as its predecessor, but adds 50 new cases to challenge would-be physicians. In Life or Death, the difficulty level is not player-selectable, but built-in. Players are promoted (or demoted) based on their performance. The game keeps track of your score and how many patients you have treated. You start out as a level I medical student with the simplest cases (e.g., cuts and scrapes). Then come the more difficult cases until you reach Level III. At this level, life-threatening cases such as strokes, heart attacks, and severe trauma are dealt with. After a receptionist nurse greets you, you will begin your first day as a staff member in the emergency room at Legacy Memorial Hospital. Once you sign in, you will have the opportunity to take a tutorial where you will learn how things are run and what is expected of you. When you are ready, click on the clipboard by the nurses' station to view the file folders of the three patients awaiting treatment.
